The Polynesia was open for 20 years, from 1961 to 1981, at Pier 51 on Seattle's waterfront. For an excellent post with more information on the Polynesia, read woofmutt's post on Tiki Central here.
This 1960's souvenir menu mailer is the first paper ephemera I've seen which shows an interior photo of the restaurant. Menus with the signature Marquesan shield logo are prominently featured in two photos, as well as a nice view of the restaurant interior with tapa carved beams, and spiral fireplace w/ lava rock reflecting pool.
